I sent all required documents signed, dated, properly identified with their cover sheet on 10.31.18 with updated and current professional licenses for 2 midwestern states. They received the documents 11.5.18, carelessly uploaded them under 'other' instead of the correct locations and then sent a false notice that my documents had failed. I had to redo all online. My time was greatly wasted by this company. More integrity and professionalism should be displayed especially since CAQH is handling the national provider data base.
